Labor at the rate the call was priced at

Hours on the call times the labor rate is the labor line, and the rate should be the one the field service pricing calculator on this site produced from your costs, not a round number from the competitor's van. An hour and a half at $90 is $135 on the worked example, shown on its own so the customer sees the hours and the rate.

Parts at your markup, tax on the parts at your rate

Parts at your cost plus your markup is what the customer pays for them: $60 marked up 40% is $84. Sales tax applies to the parts at your state's and your city's rate, shown as its own line; whether labor is taxed where you work is your state's rule, and the invoice applies the rate to the parts only and publishes none.

The trip charge on its own line, then the total

The trip or diagnostic charge is a line the customer agreed to before the visit, and it goes on the invoice as itself rather than folded into the labor. Labor plus parts plus tax plus trip is the total, $274.72 on the worked example. The paid plan keeps the invoice against the job, sends it, and takes payment by a pay link to your own account.

What should a field service invoice include?

The customer's name and service address, the date and the technician, what was done in plain words, labor hours at the rate, parts at the billed price, sales tax where it applies, the trip or diagnostic charge, the total and how to pay, and your license number where your trade requires it. The free invoice carries the figures; the paid plan adds your name, license and pay link.

Should I charge sales tax on labor?

It depends on the state: many tax parts and not repair labor, and some tax both or tax labor on certain work. The invoice applies your rate to the parts and leaves labor untaxed, which fits the common case; if your state taxes the labor too, your state's rule is the authority and your adviser knows which.
